WellSpan Emergency Services Fund
Request for Proposals
Proposals accepted on an ongoing basis.
Objective
Grants from the WellSpan Emergency Services Fund are intended to encourage nonprofit, community-based Emergency Medical Services (EMS) providers in Adams County to work toward a more coordinated, regional EMS system.
Grants may be made for the following purposes:
- Professional services, such as hiring a consultant to facilitate a merger,
- Start-up funding such as operational support in the first year,
- Development of new collaborative projects to meet the emergency medical service needs of the community.
Eligibility
- Applicants and their proposed partner organizations must be either a nonprofit and tax exempt organization, or a unit of government, or a public agency.
How to Apply
Submit:
- a copy of the IRS determination letter for each involved organization.
- a letter of intent to work toward merger, signed by all involved boards.
- the single page WellSpan Emergency Services Fund Grant Proposal Summary
- a narrative using the headings provided. Up to 3 pages, typed.
- How does this project advance a more coordinated, regional EMS system?
- Who are you partnering with?
- Why is this project needed?
- What outcomes do you expect to achieve?
- How will this project achieve more coordinated, improved and financially sustainable services?
- What are the steps you need to take to complete the project?
- How would the partnership be structured?
- Describe the costs incurred in this project.
- How will the outcomes of this project be sustained after this funding is spent?
